Case Manager - In-House Program (Female)

youth (ages 14-24) with the compassion of Jesus Christ. Our mission is to equip and empower homeless youth and young adults to achieve stability and life transformation. 

 

Position Summary 

Outreach has an opportunity for a passionate youth and young adult worker to join our team. This role builds healthy relationships with young adults (ages 14-24) experiencing homelessness. The Case Manager (Female) is responsible for working with young women to meet their basic needs, build healthy relationships, and develop long-term goals such as obtaining housing, finding employment, and building a hopeful future. 

 

Position Duties & Responsibilities

  • Youth Relationships 

    • Complete intakes for young adult women experiencing homelessness referred to Outreach 

    • Provide outreach services, and identify young adults that are living on the streets and/or in unsafe living environments 

    • Coordinate resources for youth to obtain their basic needs 

    • Work with youth to apply for housing, medical services, mental health, substance abuse, and financial resources 

    • Transport and accompany youth to appointments, as appropriate 

    • Work alongside youth in setting measurable goals and monitor progress 

 

  • Advocacy 

    • Support youth in obtaining appropriate identification documents (ID, driver’s license, etc.) 

    • Build and maintain supportive relationships with community organizations and church partners that support young adults 

 

  • Administration 

    • Maintain accurate documentation of individual and group interactions in database and provide regular reporting to supervisor 

    • Collaboratively plan and provide budget for youth events, and monitor costs related to youth outings and meals 

 

  • Outreach Staff Responsibilities 

    • Attend staff and department meetings, team trainings, 1-1 supervision with Program Director 

    • Participate in team and individual evaluations 

    • Participate in fundraiser events and other programs which further Outreach’s mission 

    • Attend local conferences and workshops that relate to homelessness

While performing the duties of this job, the employee has the potential to encounter clients with unpredictable behavior. Individuals must possess a working knowledge of trauma-informed care, de-escalation techniques, and conflict resolution skills. 

 

Supervisory Responsibility

Supervision of volunteers during specified programming. 

 

Work Environment 

This position operates in an office environment. The individual is expected to maintain personal/professional competency and works to create an environment of courtesy, respect, enthusiasm, and a positive attitude in all interactions both internal and external to the organization. This position may require occasional work outdoors with exposure to weather conditions. 

 

Position Type/Expected Hours of Work 

This is a full-time salaried position and requires the ability to be flexible in your day-to-day work schedule including evenings and weekends, for a total of 40 hours a week. 

 

Preferred Education and Experience 

  • 4+ years’ experience working with youth and young adults preferred 

  • Bachelor’s degree in field related to working with youth and young adults (social work, sociology, child & family development, etc.) preferred 

 

Additional Eligibility Qualifications 

  • Must pass a criminal background check in accordance with Outreach’s operating protocol. 

  • Must have and maintain a valid drivers’ license.

Exempt, Salaried 

 

Salary & Benefits 

$35,568 annually. Competitive health/vision/dental package and paid life insurance, 3-weeks paid vacation, 12 paid holidays, paid sick days, and paid quarterly Spiritual Formation Days. May consider increased salary, with prior extensive related training and experience.
